How to apply to become a Blue Star Distributor customer:
To apply to become an Blue Star Distributors account, you need to mail or fax
us at least three documents – the 'Dealer Application' and 'Credit Card
Authorization' forms can be found on our downloads page. The third form we need
is a copy of your Resale Tax Exception form.
All documents must be completed in their entirety, signed, dated and MAILED or
FAXED to Bluestar Distributors at:
